Should I add the same #'s listed on #14 to #16 if 16 is blank?

I assume that you are referring to form 1099-R.

 

If box 16 is blank while box 14 has an amount, then fill box 16 with the amount in box 2a. If box 2a is blank, then fill box 16 with the amount in box 1.
