Should I file form 3115 MTM accounting designation under my LLC or under my name as a member of the LLC?

Assuming your are a single member LLC, you would file using your name. A single member LLC is considered a disregarded entity by the IRS.

Thank you, would you be able to walk me through completing some of the questions after the second page of the form or is there a link where turbotax will walk me through it?

Because Form 3115 is not supported by TurboTax Online in the interview, if you are not already using the desktop version, you may wish to switch TurboTax Desktop (where it is available in Forms Mode). Consult the instructions for this form: Instructions for Form 3115.
