I need help with form 4626.

Form 4626 is the corporate alternative minimum tax, "CAMT" for large corporations - are you sure this is the Form that was requested of you? See: Form 4626.

 

The CAMT generally applies to large corporations with average annual financial statement income exceeding $1 billion.
