Form 568 is missing sides 4, 5 and 6 in the completed tax return

I used turbotax home and business deskstop version and while it generated CA form 568 for me, it's missing sides 4, 5 and 6. It only has sides 1, 2,3 and 7. Any has any ideas why that might be?

    In most cases, pages 4-6 don't apply. If they are blank, TurboTax won't include those pages when you print Form 568.
