How do I delete the 8915f4-t disaster form

You can go back to the screen that is at the end of the "Wages & Income" section of TurboTax and answer the question "No".

You can delete Form 8915 from your return as follows:

If you are using TurboTax Desktop version of TurboTax you can  do the following to delete the form:

1. In the top right corner of your TurboTax screen click on "Forms" to switch to Forms Mode.
2. In you left panel  scroll down to ‌Form 8915 and click on it
3.  in your right panel near the bottom of the form, select "Delete Form" 
4. A box will appear to confirm the deletion, Click on "Yes"

If you are using TurboTax Online version of TurboTax you can  do the following to delete the form:

1. Continue your return
2. Click on Tax Tools" then "Tools" and
3. Under "Other Helpful Links" 
4. Select "Delete a Form."
5. This will bring up all of your forms and you can 
6. Click on "Delete" next to the form you want to delete
