Trying to find my Personal HSA contributions form box 12 of my W-d.

my W2 has 12a, 12b, 12c, and 12d.

Your personal HSA contributions are any you might have made OTHER THAN what is on your W-2.

TurboTax will read Box 12 on your W-2 and transfer the information to the section it relates to.

The amount on your W-2 in Box 12 with Code W is your HSA contributions.

That amount will be accounted for by the program, you needn't enter that amount a second time.

Only enter any additional contributions you may have made personally, not through work.
