    Navigate to the form TP64.3
    Fill in the employer’s name and phone number in the tops section of the form.
    Fill in the employer’s title and date in the bottom section of the form.
    Please note that this information will not be transmitted to the government, but will allow you to continue to NETFILE your returns.
