Letter of Statement of Qualifying Retroactive Lump-Sum Payment

I received a letter and not an actual T1198 Form. It's broken out by year and Principal paid. How do I go about entering this into Turbotax without the form? I'm using the desktop version. Thanks

    If you are using TurboTax Desktop in EasyStep mode, you can search for "T1198" using the Find tool. 

     

    If you are using TurboTax Desktop in Forms mode, you can search for "T1198" by clicking on the Forms icon in the blue icon bar.
