Non-Government Annuity

I receive an annuity (not a 401K or IRA) from a company I worked for several years ago, and paid by a life insurance company. It is NOT from a government entity. Yet, upon entering my 1099-R information into TurboTax Premier 2024, the program brings me to a page asking the source of the distribution.

 

Since it is from a corporation, and not the government, none of the choices really fit. "Certain Annuity Funds" might describe it, but when I click "See Examples", they are all government examples. When I got to the State taxes, it too is treating it like a government annuity plan. Nowhere did I ever indicate that it was a government plan.

 

So how can I fix this? It doesn't look like TurboTax will allow me any other choice, but I can't see how I can file my taxes and ignore this glaring error.  Any thoughts out there?

    Does Form 1099-R have Box 7 Code D for Annuity payments from nonqualified annuities? That's unlikely as most private pension plan annuities for employees are qualified plans. Indicating this was a qualified annuity will avoid the source question.
