Question

On form 1099-R, cannot enter values in box 7 or box 15! Can anyone help?

Box 7 is just a pull-down menu. Are you saying that you can't see the triangle on the right to click on to see the pull-down list?

 

Box 15 is the State and the Payer's state ID. But if box 14 is blank, then there is not a reason to enter boxes 15 and 16. Many software vendors who print the 1099-R add the box 15 state, even when there is no withholding for that state, which is sort of pointless.
