Our 1099R says that we are exempt from state taxes. Do we still need to file a state tax form?

what state? if NC, then yes. 

 

most states begin the tax return with federal adjusted gross income or federal taxable income.  

 

The pension will be deducted from those starting figures on the state income tax form.  So it has to be reported.
