1099 misc

I receive $400 monthly reported on 1099MISC.  I am retired and this money is from my employer in lieu of spousal health insurance.  I'm having difficulty answering the questions as this situation is uncommon.  Any suggestions?

    To enter this in TurboTax, follow these steps:

     

    1. Sign into TurboTax
    2. Select Wages & Income
    3. Select Less Common Income
    4. Select Miscellaneous Income
    5. Select Other Reportable Income
    6. Enter description and amount
