How do a answer a 1099 misc for plasma donations (BioLife) when I do not know the payer's EIN or which box to put the compensation in?

It doesn't sound like you got a 1099-MISC. You may have other income.

If you don't have a W-2, it can be posted to Other reportable income.

Start at Federal

Wages & Income.
Scroll to Less Common Income.
From the drop-down select Miscellaneous Income [Select / Revisit].
Choose Other income not already reported on Form W-2 0r Form 1099.
Follow the Interview.
