or do I save my 2022 medical expenses to add on to my 2023 medical expenses for the 2023 filing?

In your case, you can wait till 2023 and include some of 2022 medical expenses on your return next year, so no medical expenses from the year after (2023) can be included in 2022, You can only combine a 12 month period from 2021 ending in 2022 if you wish to combine 2 years.
