How do I report the lump sum moving expense that I got paid from a third party company that my employer did not add onto my W-2

    You should contact your employer about it.  It should have been included in your W-2 wages.  Since 2018, employers must include moving expense reimbursements in employees’ wages.  Otherwise you need to report it as self-employment income o Schedule C, as if you had received a Form 1099-NEC.
