NJ-1040 print has a section with incorrect information: that federal extention was filed, address is foreign, etc.

New Jersey state taxes

Printed the return to review, first page of 1040 has a section listing this, most of which is untrue.

Where did this come from and how do I get rid of it?

Federal extension filed.
The address above is a foreign address.
Your address has changed.
Death certificate is enclosed.
Do not want a paper form next year.
I authorize the Division of Taxation to discuss my return and enclosures with my preparer.
NJ-1040-0 is enclosed

These are standard lines on the NJ-1040, but the return wouldn't show that these lines are valid unless there's an next to the line (see screenshot). Are you seeing an "X" next to each of the lines? If not, you're good to go.

No, I'm definitely not.  The checkboxes in front of the text are empty
