Question

I'm active duty military stationed in Colorado, but my state of residency is in Ohio. Do I have to fill anything out for buying anything without paying state sales tax?

There is a section that asks if I bought anything without paying state sales tax for items on the internet, in other states, etc. If I'm AD military do I still have to fill out this information for my Ohio state tax?

If you did not bring the items back into Ohio for use, then answer "no".  Generally, the use tax is for items that you purchase outside Ohio and use in Ohio but did not pay Ohio sales taxes on.  Here is a link on the Use Tax for Ohio.
